Manny’s Journey
I got HIV a couple of years ago. After that, I did start medication and visiting a HIV clinic in my neighborhood. It’s really just for my HIV. I don’t keep up with the rest of my health very well, and I don’t have dental insurance, so I have put off going for some problems that I know I have with my teeth.

Journey
Dental Care
I heard about the dentist at Vivent Health from a friend. I went in and needed a lot of work. I got some crowns and other stuff. It was something I needed. It took several appointments, so I got to know some of the other things they do. Matt, one of the case managers talked to me, and I made an appointment to talk more.

Journey
HIV Case Management Services
I saw Matt again, he’s a good person. We talked and I know he gets me and sees all of me. It made me realize I am not taking care of myself. That I should get help for my body and my mind. I’m carrying a weight on my shoulders for the past couple of years that I can’t shake.

Journey
Medical Care
This is amazing to me. My dentist is talking to my doctor and Matt is watching my back. I am feeling like this is going to change my life. For the first time in my life, I know things about my body I never knew. I am starting to take better care of myself. And feel like people care about me.

Journey
Pharmacy
I go to the pharmacy in the building where my doctor and dentist are. I don’t have to travel around the city. I always felt like people were looking at me when I got HIV meds. It’s that weight I carry. Now it’s so private and easy. I haven’t missed an appointment or taking my meds. That was a problem before.

Journey
Behavioral Health Care
I am feeling better about myself. But I need some help with things that have been dragging me down. I don’t feel I can talk about HIV with anyone. Not my family or my friends. Definitely not at work. It’s not accepted. Matt gets me to work with Francis, who is really cool. I know this is going to work out, I can feel it.

Journey
Other Services: Food Pantry, Legal Services, Housing Programs and Prevention
Matt made me aware that I could get two weeks of groceries if I need them and help on any legal things with my landlord or work. And I know where to go to help keep my relationships safe in the future. It is all good.
